Consolidate duplicate SubmissionForm components (quest/ vs submission/ directories)

Background

Both components/quest/SubmissionForm.tsx and components/submission/SubmissionForm.tsx exist with the same name in different directories — high risk that contributors edit or import the wrong one.

Implementation Plan

• Compare both implementations for behavioral differences
• Merge into a single component in the more appropriate directory (likely components/submission/)
• Update all imports; delete the redundant file
• Add a test for the consolidated component covering both prior use cases

Acceptance Criteria

• Only one SubmissionForm component exists
• All call sites updated
• Test coverage added
