[Feature]: Document manual installation

Feature Description

I don't use VS Code or Cursor, and am not really familiar with Node projects, so I couldn't quickly figure out how to install this. Your instructions jump straight to assuming it's already installed and how to integrate it with my agent (Codex, in my case). But how do I build and install the binary on my system for Codex to use?
