Document symbol request reporting seems broken

• Does not update when changes are made to the file
• Names of objects are duplicated as symbols next to them (fixed)
• Will, on occassion, disappear and become unavailable

Uncertain if root cause is actually in document symbols or workspace symbols

vscode uses document symbols for this, workspace symbols seems to be a different thing
