adds preserve_time_ns_as_column function which prevents time_ns indicies from being duplicated. Also uncomments binning and post-refraction fits.

sliderule_io_reformat primarily defines the DataRequest Object and KdConfig objects which handle calls to sliderule and user input respectively.
