VMT: Add the Vulnerability Management Team charter - #460

The old VMT process document has been marked as outdated, but the project still lacks a written definition of the team itself. Add a charter that states what the VMT is responsible for (and, explicitly, what it is not responsible for), how members are selected and appointed by the Architecture Committee, and under which circumstances a member can be removed. Let's also link it from the README so it is discoverable next to the rest of the vulnerability handling documentation.
