Pin the reviewed permission-review safety foundation as a fourth release-sync patch.

This also adds the plugin-trigger and tool-registry regression tests to candidate verification and makes strict metadata validation derive its patch count from the pinned branch list.

Validated locally with bash -n, ShellCheck, focused tests, typechecks, Prettier and an independent security/merge-safety review.
