Summary
Why
PR #91 merged 55 seconds before Codex submitted a valid finding, and PRs #89/#90 had no Codex review evidence. Fast Checks were incorrectly treated as sufficient merge readiness. This PR makes review an independent, machine-checked release gate.
Validation
Scripts/test_pr_review_gate.sh./Scripts/lint.sh lintScripts/check_pr_review_gate.sh 91rejects the merged PR, missing clean current-head result, and unresolved threadCloses the process gap exposed by #91.
